For Large Group plans only: Our Connect network provides your employees with quality care from Kaiser Permanente’s high-performing physician group — the Washington Permanente Medical Group. It's an optimized network, designed to support our Kaiser Permanente Virtual Plus™ plans with several virtual care delivery options plus in-person care when needed.

Core HMO

With Access PPO, you can choose from an extensive network of preferred primary and specialty care providers, including our exclusive medical group at Kaiser Permanente medical facilities. You also have the option to get care through our regional and national networks.

Summit PPO

Value and choice are highlights of our 3-tier Summit PPO plans. Your employees get the best value when they choose high-quality care from Kaiser Permanente doctors and preferred contracted providers. And when choice is most important, they have access to more than 1 million in-network health professionals anywhere in the United States.
  • 3-tier plans offering extensive choice in providers and costs
  • 16 standard (non-customizable) plans
    • 4 health savings account (HSA) plans
    • 6 non-HSA plans
    • 6 welcome plans
  • Shared in-network deductible and out-of-pocket limits between tier 1 and tier 2, which help lower costs
  • Offered as a sole product or alongside a second Kaiser Permanente health plan product

Health360

Developed in collaboration with Delta Dental of Washington, our Health360 plan offers integrated dental benefits for whole body health. By combining a Kaiser Permanente health plan with a Delta Dental PPO plan, members enjoy access to high-quality Kaiser Permanente medical care and the nation’s largest contracted network of dentists in a single package. Enhanced benefits for members include:

  • Extra annual dental care visits — at no additional cost — for members with qualifying health conditions, including heart disease, diabetes, and pregnancy
  • Increases to their annual dental plan maximum if they receive dental preventive care
Benefits for employers include:
  • Bundled pricing, with one combined quote for a medical and dental package
  • Single point of contact for many aspects of medical and dental plan administration
  • Potential cost savings due to improvements in employees’ overall health3
