Access PPO

Our Access PPO plan gives your employees maximum choice with access to one of the state’s largest preferred provider networks — and it’s the only PPO plan that offers access to our high-performing health system at Kaiser Permanente medical facilities.

  • Kaiser Permanente clinicians, medical facilities, and pharmacies
  • More than 26,000 additional network providers1
  • Access to most providers and designated pharmacies in our service area, including UW Medicine, Swedish Physicians, MultiCare, CHI Franciscan, PeaceHealth, Providence, and more
  • First Choice Health network providers for Washington, Oregon, Alaska, Montana, and Idaho
  • First Health network providers for all other states
  • OptumRx network pharmacies nationwide
  • Access to any other licensed provider at the out-of-network benefit level

Elect PPO

Our Elect PPO3 plans offer the choice and flexibility of in-network care from the high-performing Kaiser Permanente provider network within our Washington service area, and from the broad provider networks of First Choice Health and First Health outside our Washington service area.

  • Kaiser Permanente clinicians, medical facilities, and pharmacies
  • More than 16,000 additional network providers1
  • First Choice Health network providers for in-network care outside the service area counties in Washington, Oregon, Alaska, Montana, and Idaho
  • First Health network providers for in-network care in all other states

Health360

Developed in collaboration with Delta Dental of Washington, our Health360 plan offers integrated dental benefits for whole body health. By combining a Kaiser Permanente health plan with a Delta Dental PPO plan, members enjoy access to high-quality Kaiser Permanente medical care and the nation’s largest contracted network of dentists in a single package. Enhanced benefits for members include:

  • Extra annual dental care visits — at no additional cost — for members with qualifying health conditions, including heart disease, diabetes, and pregnancy
  • Increases to their annual dental plan maximum if they receive dental preventive care
Benefits for employers include:
  • Bundled pricing, with one combined quote for a medical and dental package
  • Single point of contact for many aspects of medical and dental plan administration
  • Potential cost savings due to improvements in employees’ overall health4

Value and choice are highlights of our 3-tier Summit PPO plans. Your employees get the best value when they choose high-quality care from Kaiser Permanente providers and preferred contracted providers. And when choice is most important, they have access to more than 1 million in-network health professionals anywhere in the United States.

  • 3-tier plans offer extensive choice in providers and costs
  • 10 standard, non-customizable plans, with 4 health savings accounts (HSA) plans and 6 non-HSA plans
  • Shared in-network deductible and out-of-pocket limits between Tier 1 and Tier 2 help lower costs
  • Offer as a sole product, or alongside a second Kaiser Permanente health plan product
  • Self-funding and risk-sharing funding arrangements are available
